Our Health Risk Coaches motivate individuals to make better choices about nutrition and lifestyle. We believe understanding your risk and the impact risks have on healthcare costs is key to developing a sound strategy for total population management. With the goal of reducing costs associated with risk factors, our coaching program assists employers in identifying at-risk populations through biometric data, as well as lifestyle risk factors. At HC21, we begin by working with employers to design a customized face-to-face program that fits your needs and budget.

  • An evidence-based approach to discovering triggers that make behavior change sustainable
  • Quarterly reports that track key indicators such as "readiness to change"
